Key Responsibilities

  • Deliver and coordinate patient and caregiver care tailored to individual needs.

  • Empower patients and caregivers through education, fostering independence and understanding.
  • Adhere to established protocols and guidelines for exceptional hospice care delivery.
  • Communicate effectively with patients and families, providing clear information and addressing inquiries.
  • Collaborate closely with the interdisciplinary team (IDG), patients, caregivers, and healthcare professionals to ensure integrated care.

Qualifications

  • Diploma or Associate's Degree in Nursing (Bachelor's Degree preferred).
  • Current, active, and unrestricted compact Registered Nurse (RN) license.
  • Valid CPR certification (CHPN preferred).
  • Minimum of 1 year of experience in inpatient care alongside 2 years of nursing experience in relevant fields, specifically with patients facing life-limiting illnesses.
  • Proficiency in computer applications is required.
