PACE Registered Nurse (RN) | Remote | Up to $55/hr | WFH

UnitedHealth Group is seeking a dedicated PACE Registered Nurse (RN) to provide high-quality, patient-centered care for seniors enrolled in the Program of All-Inclusive Care for the Elderly (PACE). In this role, you will support patients remotely, ensuring their healthcare needs are met while promoting independence and well-being. Responsibilities

  • Assess, monitor, and coordinate patient care for PACE participants.
  • Develop individualized care plans in collaboration with an interdisciplinary team.
  • Educate patients and families on disease management, medication, and overall wellness.
  • Serve as a primary point of contact for patient concerns, ensuring timely interventions.
  • Work closely with physicians, social workers, and other healthcare professionals to optimize patient outcomes.
  • Maintain accurate and timely electronic medical records (EMR).
  • Ensure compliance with federal, state, and company healthcare regulations.

Requirements

  • Active RN license in the state of practice.
  • Minimum of 2 years of nursing experience, preferably in geriatric care, home health, or case management.
  • Previous experience in PACE, managed care, or care coordination is a plus.
  • Strong communication, organizational, and critical thinking skills.
  • Comfortable using technology and electronic medical records (EMR).
  • Ability to work independently while collaborating with a remote healthcare team.
